A Professional Geriatric Care Manager (PGCM) is a health and human services specialist who is a guide, advocate and resource for families caring for older relatives. The PGCM may be trained in any one of a number of fields relating to long-term care.
The PGCM assists older adults and persons with disabilities reach their maximum functional potential. The individual’s independence is encouraged, provided safety and security concerns are addressed.
PGCM’s have extensive knowledge about costs, quality, and availability of resources in their communities.
